2988842629-2 Apply for this job now Job Description BoldAge PACE seeks a compassionate Physical Therapist to provide comprehensive rehabilitation services to older adults in our innovative PACE program. You will assess, plan, and deliver individualized treatment to improve mobility, function, and quality of life across clinic, home, and community settings. Working within an interdisciplinary team, you'll collaborate closely with physicians, nurses, social workers, and caregivers to coordinate care. Our collaborative, learning-focused culture offers opportunities for professional growth, while emphasizing patient-centered care, safety, and staff well-being. Responsibilities Evaluate participants' functional status, mobility, strength, balance, and pain using standardized assessments Develop and implement individualized, evidence-based PT treatment plans for older adults Provide therapeutic exercise, gait training, balance and fall-prevention interventions, and functional mobility training Educate participants and caregivers on home exercise programs, safe mobility, and use of assistive devices Collaborate with an interdisciplinary PACE team to coordinate and adjust care plans Document assessments, goals, and progress notes accurately in the EMRContribute to discharge planning and recommendations for equipment or additional services Participate in quality improvement initiatives and clinical case conferences Adhere to regulatory, ethical, and safety standards for PACE and PT practice Mentor or support team members and assist with staff/patient education as needed Required Skills Geriatric physical therapy Functional and fall-risk assessment Therapeutic exercise prescription Gait and balance training Neuromuscular re-education Orthopedic and post-acute rehab techniques Use and fitting of assistive devices and durable medical equipment Manual therapy techniques Pain management strategies for older adults Electronic medical record (EMR) documentation
