Full Job Description Physical Therapist Position Overview:

Develop individualized treatment plans to help residents regain independence, prevent decline, and safely participate in daily activities while enhancing quality of life.

Key Responsibilities:

Assessing and interpreting patient evaluations and test results. Developing treatment plans using a variety of treatment techniques. Creating personalized rehabilitation programs for patients. Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.

Preferred Qualifications:

Active/Valid Illinois license in Physical Therapy SNF/Long term care experience preferred. Current knowledge of treatment practices Knowledge of PCC Electronic Documentation System preferred.
