About Formed Formed Physical Therapy & Wellness is a rapidly growing cash-based orthopedic practice serving active adults throughout Northern Indiana. We specialize in helping runners, lifters, active professionals, and motivated adults overcome pain, return to sport, and optimize performance. What You'll Do Evaluate and treat orthopedic patients using evidence-informed care Develop individualized plans of care focused on meaningful patient goals Deliver one-on-one 60-minute treatment sessions Build lasting relationships with patients and become their trusted movement expert Collaborate with teammates to continually improve the patient experience Participate in occasional workshops, community events, and networking opportunities Contribute ideas that help our practice continue to grow What We're Looking For We care far more about attitude than years of experience.
