The Patient Made It Home. Now Make Sure They Can Stay There. A patient can be medically ready for discharge and still be nowhere near ready for the realities of being home. The hospital hallway was flat. Their home has three steps at the entrance. They transferred safely with staff nearby. At home, their spouse is the only person available to help. They walked 100 feet with a walker in rehab. Now they need to navigate a narrow bathroom, get out of their own bed, reach the kitchen, and somehow manage everything they used to do before hospitalization. Getting discharged home is only the beginning. Home Health Care, Inc. is hiring an experienced Physical Therapist (PT / DPT) to complete Start of Care admissions throughout the Twin Cities Metro and help determine what patients actually need to remain safe and successful at home. During a Start of Care visit, you're not evaluating mobility in a controlled clinical environment. You're seeing exactly how the patient functions where they actually live. Can they safely get out of their favorite chair? Navigate the stairs? Reach the bathroom at night? Use their walker in a tight hallway? Get into bed? Manage the entrance to their home? You're identifying the real-world mobility barriers that can determine whether recovery continues or starts moving backward. Maybe the patient is much more fatigued at home. Maybe they're afraid to walk without someone nearby. Maybe their walker doesn't work well in the space. Maybe their spouse doesn't know how to safely assist with a transfer. Your assessment helps identify those barriers early and determine what needs to happen next. That may include Physical Therapy, Occupational Therapy, skilled nursing, additional equipment, caregiver education, fall-prevention strategies, or another resource entirely. The goal is bigger than completing an evaluation. It's giving the patient the best possible chance of succeeding at home. What You'll Be Doing Complete comprehensive Start of Care Physical Therapy evaluations in patient homes Assess gait, balance, strength, endurance, transfers, mobility, and fall risk Evaluate how patients function within their actual home environment Complete OASIS and admission documentation accurately and within required timeframes Identify barriers that could jeopardize a safe recovery at home Develop individualized plans of care based on clinical findings, physician orders, and patient goals Recommend appropriate assistive devices and durable medical equipment Provide initial education on mobility, fall prevention, home exercise, and safe functional movement Assess caregiver ability and identify education or support needs Collaborate with OT, SLP, nursing, physicians, social work, and other interdisciplinary clinicians Communicate significant clinical findings promptly Complete timely electronic documentation and maintain regulatory compliance You See Things Differently When You Evaluate the Patient at Home A fall-risk score tells you something. Seeing the loose rug between the patient's recliner and bathroom tells you something else. A strength assessment matters. So does realizing the patient has to climb eight stairs to reach their bedroom. Knowing someone has caregiver support is helpful. Watching that caregiver attempt a transfer gives you an entirely different picture. Home health lets you connect clinical findings to the environment where recovery actually has to happen. That's why we're looking for a PT with the experience and clinical judgment to recognize not only what a patient can do, but whether what they're doing is actually safe and sustainable. What You Bring Current Minnesota Physical Therapist license in good standing Current CPR certification Valid driver's license and reliable transportation Ability to travel throughout an approximately 75-mile service radius Strong comprehensive assessment and critical-thinking skills Excellent communication and documentation abilities Ability to independently manage a field-based schedule Strong interdisciplinary collaboration skills Experience That Will Help You Stand Out Previous home health experience OASIS experience Start of Care or admission experience Knowledge of Medicare home health requirements Experience with orthopedic, neurological, cardiac, pulmonary, geriatric, or medically complex populations Weekends Are Optional. Holidays Are Yours. Your regular schedule is during the week, with Start of Care visits paying $100 per visit plus mileage reimbursement . If you'd like additional earning opportunities, you can choose to complete weekend admissions at the higher $120 per-visit rate when visits are available and your schedule allows. Weekend visits are optional, and there are no holiday requirements. A Successful Discharge Shouldn't End With Another Hospital Admission The patient has already done the hard work of getting home. Now they need clinicians who can recognize what could prevent them from staying there. As a Start of Care Physical Therapist , you'll help identify those risks, establish the right plan from the beginning, and give patients and caregivers the tools they need to move forward safely.
