In Southern Minnesota, your patient may live miles outside of town. They may no longer drive. Their spouse may be unable to safely get them into the car. Legacy Home Care and Home Health Care, Inc. are expanding our Physical Therapy coverage throughout Southern and Southwestern Minnesota , bringing skilled therapy directly to patients who need it. You'll be on the road throughout an assigned portion of Southern or Southwestern Minnesota, seeing patients individually and bringing skilled care directly into their homes. That travel serves a purpose. Every mile takes therapy closer to someone who may otherwise struggle to access it. You'll have the independence to manage a productive regional caseload while still having the resources of an experienced home health organization and interdisciplinary clinical team behind you. What You'll Do Provide in-home Physical Therapy evaluations and treatment visits Complete Start of Care assessments and admissions Develop individualized plans of care Address strength, balance, gait, transfers, mobility, endurance, and functional independence Identify fall risks and barriers to safely remaining at home Educate patients, families, and caregivers Supervise and collaborate with Physical Therapist Assistants Coordinate care with nurses, physicians, therapists, and other interdisciplinary professionals Monitor patient progress and modify plans of care as appropriate Complete accurate and timely documentation and OASIS assessments Great PT Experience Translates You don't need to spend years in home health before joining us. We're interested in Physical Therapists coming from Acute Care, Inpatient Rehabilitation, Outpatient Orthopedics, Skilled Nursing, Transitional Care, Senior Living, Hospital-Based Rehabilitation, Geriatric Therapy, and Home Health . Home health and OASIS experience are preferred, but strong clinical judgment, productivity, organization, independence, and communication can translate extremely well into the field. What We're Looking For Current Minnesota Physical Therapist license PT or DPT Strong clinical assessment and decision-making abilities Ability to independently manage a productive regional caseload Comfort traveling throughout an assigned Southern or Southwestern Minnesota territory Valid driver's license and reliable transportation Excellent patient and caregiver communication Home health experience preferred OASIS experience preferred Availability for at least 2 weekdays / 12 visits per week Your Drive Can Be the Reason Someone Gets Care For the right PT, regional home health offers something different: your expertise reaches people where access to therapy isn't always easy.
