Discharge Isn't the Finish Line. Sometimes It's Where the Hard Part Begins. The patient made it through surgery. Or the hospitalization. Or inpatient rehab. They've been cleared to go home. Then they walk through their own front door and realize recovery looks very different without hospital rails, therapy equipment, wide hallways, and clinicians nearby. That's where you come in. Home Health Care, Inc. is hiring a Physical Therapist (PT/DPT) to serve patients throughout Bethel, Cedar, East Bethel, Oak Grove, and St. Francis . You'll help patients bridge the gap between being medically ready to go home and actually being able to function there safely. Help Patients Finish the Recovery They Started Somewhere Else A successful discharge doesn't necessarily mean someone is back to normal. Your patient may still be afraid of the stairs. Their spouse may not know how to safely assist with transfers. They may be using a walker for the first time. Getting from the bedroom to the bathroom may take everything they have. Or they may simply be terrified of falling again. As their Physical Therapist, you'll meet them in that moment and help turn "you're ready to go home" into "I can actually do this at home." Your work may mean helping someone: Safely navigate the stairs in their own home Regain strength after hospitalization Improve balance following repeated falls Learn to use a walker safely Transfer independently from their own bed or chair Rebuild endurance for everyday activities Give a spouse or caregiver confidence assisting with mobility Progress toward functioning independently again What You'll Do Perform comprehensive in-home Physical Therapy evaluations Develop individualized plans of care and functional goals Provide skilled interventions addressing strength, balance, gait, mobility, transfers, and endurance Evaluate fall risks and home safety concerns Recommend appropriate equipment and modifications Educate patients and caregivers on safe mobility Monitor progress and adjust treatment plans Provide appropriate supervision and direction to PTAs Collaborate with physicians, nurses, OTs, SLPs, and other clinicians Complete accurate and timely clinical documentation Provide care throughout Bethel, Cedar, East Bethel, Oak Grove, and St. Francis Bring Your PT Experience Into the Next Stage of Recovery We're interested in Physical Therapists from Acute Care, Inpatient Rehabilitation, Outpatient Orthopedics, Skilled Nursing, Transitional Care, Senior Living, Long-Term Care, Hospital Rehab, Home Health, and other therapy settings . If you've worked with patients before they returned home, this is an opportunity to see what happens next . You'll need an active Minnesota Physical Therapist (PT) license , a valid driver's license and reliable transportation, strong clinical judgment, excellent communication skills, and the ability to independently manage a field-based schedule. Home health and OASIS experience are preferred, but not required. Getting Home Is One Milestone. Getting Their Life Back Is Another.
