Hospice:
+ Experience true work/life balance with flexible full-time, part-time, and per diem schedules + Perform 1:1 direct care in client homes + Hands-on training supplemented with virtual learning + 24/7 clinical support as part of a larger interdisciplinary care team + Advance your career with clinical and business leadership tracks What you'll do: Visit a variety of clients in their own home, helping them recover from an injury, illness, surgery, or hospitalization. Responsibilities for a PT - Physical Therapist include: + Make home visits to clients in designated geographic territories. + Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments. + Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents. + Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions. + Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate. + Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living. + Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.
What you should have:
+ Active and current PT license + One year of clinical experience preferred + Previous home health care experience welcomed, but not required.
