Job Overview We are seeking a dedicated and skilled Physical Therapist to provide high-quality patient care on a PRN (as-needed) basis within our Skilled/Long-term Care community. The ideal candidate will possess a strong background in physiotherapy, with expertise in developing treatment plans, conducting assessments, and collaborating with healthcare professionals to optimize patient outcomes. This role offers flexibility while maintaining a focus on delivering compassionate, evidence-based therapy to geriatric patient populations, Responsibilities Conduct comprehensive evaluations of patients to determine their physical therapy needs, including assessments of physiology, anatomy, and medical history. Develop personalized treatment plans that align with clinical documentation standards and best practices in physiotherapy. Administer therapeutic interventions such as trigger point therapy, patient rehabilitation exercises, and mobility training tailored to individual goals. Collaborate effectively with physicians, nurses, and other healthcare professionals to coordinate discharge planning and ensure continuity of care across settings. Maintain accurate electronic health records (EHR) and electronic medical records (EMR) documentation in compliance with regulatory standards. Educate patients and their families on treatment procedures, home exercises, and community care options to promote recovery and independence. Assist in outpatient clinic operations or home health visits as needed, providing exceptional patient care in various environments. Stay current with advancements in clinical practices related to orthopedics, neurology, geriatrics, pediatrics, and outpatient rehabilitation. Qualifications Valid license or certification to practice as a Physical Therapist in the applicable jurisdiction. Prior experience working in outpatient clinics, inpatient hospital settings, skilled/long-term care, or home health environments is highly desirable. Demonstrated proficiency in discharge planning, treatment plan development, and clinical documentation standards. Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques such as trigger point therapy. Experience working with geriatric patients, pediatric populations, or neurological conditions is preferred. Familiarity with EMR systems and healthcare documentation protocols to ensure accurate recordkeeping. Excellent client communication skills with the ability to educate patients effectively about their care plans.
