Key Responsibilities:
Conduct initial and ongoing assessments of mobility, strength, and function. Administer, evaluate, and interpret tests to set functional goals and outcomes. Provide written evaluations and treatment plans outlining needs, goals, and progress. Deliver physical therapy services following physician orders, maintaining documentation, and modifying treatment as needed. Plan discharge care, including patient and caregiver education on home exercises. Coordinate with healthcare team members and document according to policies, ensuring compliance and minimal supervision. Supervise clinical personnel (PTA, HHA) as applicable and complete assessments within state regulations.
Qualifications & Skills:
Degree in Physical Therapy from an accredited program. Current, unrestricted state license as a Physical Therapist. Valid CPR certification and a current driver's license.
