One-on-one outpatient visits in the home — DDD, Medicare Part B, commercial PPO, and cash pay. Not home health. We bring one-on-one physical, occupational, and speech therapy into homes, assisted living, group homes, and day habilitation programs across Bergen, Hudson, Passaic, and Middlesex Counties, New Jersey.
Caseload mix:
NJ DDD / delayed developmental disability, Medicare Part B, commercial PPO, and cash-pay visits. This is outpatient therapy in the home or day hab — not a Medicare home-health agency. No OASIS. There is no OASIS documentation and no homebound requirement. You treat like a skilled outpatient clinician, in the place the patient actually lives or spends the day. True outpatient clinical work — not home health, not a 12-patient clinic mill A tight town-based route, not a 90-minute windshield day You set availability; no corporate unit quota 1:1 sessions in the patient's real environment Direct access to the owner-clinician, not a call center Responsibilities Evaluate and treat adults and older adults using the actual home: stairs, bathtub, kitchen, sidewalks Treat 1:1 in private homes, assisted living, or group homes in the posted towns Caseload may include DDD / developmental disability, Medicare Part B, commercial PPO, and cash-pay visits Write timely outpatient (Part B) notes — no OASIS Coordinate with families, physicians, and the rest of our therapy team Supervise PTAs on shared patients when assigned Requirements Current New Jersey Physical Therapist license (or compact privilege to practice in NJ) Doctor of Physical Therapy or equivalent entry-level PT degree Valid driver's license, reliable vehicle, and auto insurance Ability to treat independently in the community and transfer or guard patients safely Preferred Geriatric, neurologic, or developmental-disability experience Spanish or Korean Ability to cover more than one listed town Pay and benefits W-2 per diem up to $75 per hour, or 1099 at a higher contracted rate W-2 paid biweekly; $500-$1,000 visit advance after your first completed week (recouped on paycheck #2) 1099 paid weekly after signed notes Mileage at the IRS standard rate on the W-2 track Company-paid professional liability on the W-2 track You publish the days you can work First in line when a part-time or full-time W-2 seat opens
Pay:
W-2 is biweekly (work two weeks, paid the following Friday). New W-2 per diem clinicians can take a $500-$1,000 visit advance after the first completed work week; it is deducted from paycheck #2. 1099 contractors are paid weekly after signed notes.
