Job Description

Mid-American Therapy is looking for a progressive, dynamic and committed Physical Therapist to join our home health team and contribute to our mission. The right candidate will work with Physical Therapy Assistants to carry out the plan of care, as well as collaborate with other health care professionals to ensure the best treatment plan for the patient. Responsibilities Identify and meet patient goals and needs Develop care plans using a variety of treatment techniques Provide quality, personalized and evidence-based care and proven interventions Motivate patients during visit to promote optimal function and outcomes Promote clients' quality of life by improving ROM, strength, flexibility, balance, and coordination Consult and practice with other health professionals Evaluate, monitor, modify and communicate progress of patient Document patient care services in a timely manner Skills Ability to manage patients with different types of personalities Current knowledge of treatment practices Drive to continually learn and grow Professional behavior Excellent interpersonal communication skills Basic computer skills for our documentation software Valid license to practice physical therapy in the state of

$40.00 - $65.00 per hour
