An opportunity is available for an experienced and compassionate Home Health Physical Therapist (PT) to deliver skilled care directly to patients in their homes. This contract position offers the chance to foster patient independence, enhance mobility, and make a meaningful impact through individualized therapy.

Schedule and Assignment Details:

Full-time, 13-week contract Monday Friday, 8:00 AM 5:00 PM Approximately 25-32 points per week (typically 5-7 patient visits daily) Field-based role serving patients in their homes Daily travel within assigned territory (mileage reimbursement included) Electronic documentation using Home Care Home Base (HCHB) and OASIS assessments as required

Responsibilities:

Complete thorough physical therapy evaluations, including OASIS, to assess patient needs and develop individualized care plans Implement therapeutic interventionssuch as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall preventionto maximize functional mobility and patient safety Educate patients and caregivers on home exercise programs, use of assistive devices, and safety strategies Monitor and document patient progress, modifying plans as necessary and maintaining compliance with care standards Collaborate effectively with a multidisciplinary team including nurses, OTs, SLPs, social workers, and home health aides Participate in case conferences, meetings, and ongoing professional development

Qualifications:

Active, unencumbered Physical Therapist (PT) license for the state (required) Minimum one year of recent home health PT experience, including fieldwork Experience with OASIS and Home Care Home Base (HCHB) documentation strongly preferred Solid clinical assessment, decision-making, and time management abilities Reliable transportation, drivers license, and insurance required Benefits include weekly pay, options for medical/dental/vision insurance, 401(k) retirement plan choices, round-the-clock team support, mileage reimbursement, and potential for contract extensions.
