Job Description

Provide skilled Physical Therapy services, on a intermittent basis, to patients, in their homes, in accordance with physician orders Provide initial and ongoing, comprehensive assessment of the patients needs, including Oasis Assessments Assist in Development and implementation of patient interdisciplinary plan of care as it pertains to physical therapy Establish goals, based on the patient's current level of functioning, and potential for improvement and performs re-evaluations, as indicated Summary As a Physical Therapist, you will be essential in delivering high-quality patient care and rehabilitation services to individuals recovering from injuries or managing chronic conditions. You will utilize your expertise in anatomy, physiology, and medical terminology to develop effective care plans. Your core skills in physical therapy will be complemented by premium skills in discharge planning and care, particularly in Home Care patients. Additionally, your experience in home health settings will enhance our commitment to patient-centered care, ensuring optimal recovery outcomes for our diverse patient population. Qualifications (Suggested) Strong patient care abilities In-depth knowledge of physiology and anatomy Proficiency in medical terminology and physical therapy techniques Experience in discharge planning and creating care plans Familiarity with home care settings Background in home health, outpatient, and inpatient care Understanding of trigger point therapy and orthopedics Experience working with geriatric patients and community care systems

$70.00 - $140.00 per hour Expected hours: 10.0 - 20.0 per week
