Job Description

Springfield Nursing in Springfield, OH is seeking a PRN supervising Physical Therapist to join our growing In-House therapy team. This is an excellent opportunity for a compassionate and motivated clinician looking to make a meaningful impact in a collaborative, skilled nursing and rehabilitation setting. Need consistent coverage for approximately 4-6 hours a week Schedule is flexible Springfield Nursing and Independent Living is a 90-bed senior living community offering assisted living and memory care. Set in the "Champion City," the facility sits in a region rich with industrial history and outdoor beauty.
Essential Duties & Responsibilities:
Plan treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing Assure all treatment is delivered in accordance with an established plan of care Provide clinical support and instruct patients, families, and caregivers Monitor patient response to treatment intervention Complete required forms and documentation in accordance with company policy and state/federal regulations Consulting with patients to learn about their physical condition. Assessing and interpreting patient evaluations and test results. Developing treatment plans using a variety of treatment techniques. Creating personalized fitness-oriented health care programs for patients. Administering medically prescribed physical therapy treatments to relieve pain and improve mobility. Advising patients on exercise techniques. Advising patients and their families about in-home treatment options. Providing educational information about injury prevention, ergonomics and ways to promote physical health. Consulting and collaborating with other healthcare professionals. Documenting patient care history. Complying with rules, regulations, and procedures.
Qualifications:
degree from an accredited Physical Therapist program. Current PT license in the state of employment. CPR certification required. Previous experience in skilled nursing, rehab, or long-term care preferred. Strong understanding of rehabilitation principles, restorative care, and geriatric populations. Knowledge of Medicare Part A, Part B, and managed care preferred. Excellent communication and teamwork skills.
