Job Summary:

We are seeking a skilled and motivated Physical Therapist to join our team and make a positive impact on the lives of our patients. The Physical Therapist will be responsible for assessing, planning, and implementing individualized therapy programs for residents in a skilled nursing facility. The role involves working closely with a multidisciplinary team to deliver high-quality rehabilitative care that promotes functional independence and overall well-being. This position offers flexibility with scheduling and is ideal for those looking to work on an as-needed basis.

This position operates in a clinical healthcare environment.

Responsibilities:
Patient Assessment:

Evaluate patients' physical conditions, mobility, and functional abilities to develop comprehensive treatment plans tailored to their needs.

Treatment Planning:

Create and implement personalized therapy programs that address specific goals, improve mobility, alleviate pain, and enhance overall physical function.

Therapeutic Interventions:

Administer therapeutic exercises, manual therapy techniques, and modalities to help patients achieve their rehabilitation goals.

Progress Monitoring:

Regularly assess and document patient progress, adjusting treatment plans as necessary to optimize outcomes.

Patient Education:

Educate patients and their families on exercise techniques, safety measures, and home care strategies to support ongoing recovery and maintenance.

Collaboration:

Work closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and address any issues that may impact patient progress.

Documentation:

Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries in compliance with facility policies and regulatory requirements.

Compliance:

Adhere to all safety and infection control protocols, as well as facility policies and procedures, to ensure a safe and effective therapeutic environment.

Qualifications:
Education:

Doctoral or Master's degree in Physical Therapy from an accredited institution.

Licensure:

Current state licensure as a Physical Therapist.

Experience:

Previous experience in a skilled nursing or long-term care setting preferred, but not required.
