Job Description

Job Summary We are seeking a dedicated and compassionate Mobile Physical Therapist to join our team. The ideal candidate will be responsible for developing and implementing personalized treatment plans to improve patients' mobility, strength, and overall well-being In their homes. This role involves providing in-home outpatient concierge visits, offering convenient and personalized care tailored to each patient's needs. The Physical Therapist will play a vital role in helping patients recover from injuries, surgeries, or chronic conditions through expert therapy interventions.

Harleysville, Souderton, Telford, Schwenksville, Green Lane Responsibilities Conduct comprehensive assessments of patients' physical conditions to determine appropriate treatment strategies. Develop individualized treatment plans aimed at improving mobility, reducing pain, and enhancing functional abilities. Deliver in-home outpatient concierge therapy visits, ensuring patient comfort and safety during each session. Implement therapeutic exercises, manual therapy techniques, and other interventions as outlined in the treatment plan. Monitor patient progress regularly and adjust treatment plans accordingly to achieve optimal outcomes. Educate patients and their families on exercises, injury prevention, and health maintenance strategies. Maintain accurate documentation of patient evaluations, treatment sessions, progress notes, and other relevant records in compliance with healthcare regulations. Collaborate with healthcare teams and other professionals to coordinate patient care when necessary. Skills Proven experience in physical therapy with a strong understanding of treatment plan development. Excellent interpersonal skills with the ability to build rapport with diverse patient populations. Strong assessment and clinical reasoning abilities to tailor treatments effectively. Ability to perform manual therapy techniques and facilitate therapeutic exercises safely and effectively. Exceptional organizational skills for managing multiple patient visits and maintaining detailed documentation. Flexibility to provide in-home outpatient services with a focus on patient-centered care. Knowledge of healthcare regulations, safety protocols, and best practices in physical therapy. Pay:
$40.00 - $50.00 per hour Expected hours: 10.0 - 40.0 per week
