Responsibilities Conduct comprehensive physical therapy evaluations in patients' homes Develop and implement individualized treatment plans and goals Provide therapeutic exercises, gait and balance training, and functional mobility interventions Educate patients and caregivers on home exercise programs and safety strategies Collaborate with physicians, nurses, and other clinicians to coordinate care Accurately document assessments, treatments, and progress in EMR systems Monitor, reassess, and adjust interventions based on patient response Ensure compliance with Medicare, payer, and home health regulatory requirements Participate in case conferences and quality improvement initiatives Maintain professional development and uphold clinical best practices Required Skills Physical therapy evaluation and treatment planning Home health rehabilitation techniques Gait training and balance rehabilitation Manual therapy techniques Therapeutic exercise prescription Wound care and pain management basics Electronic medical record (EMR) documentation Patient and family education Knowledge of Medicare/home health regulations Interdisciplinary care coordination
