Western Hills Healthcare Center is seeking a compassionate and motivated Per Diem Physical Therapist (PT) to join our dedicated rehabilitation team. This flexible PRN opportunity is ideal for a therapist who is passionate about helping residents regain strength, mobility, and independence while delivering exceptional, resident-centered care. As a valued member of our interdisciplinary team, you'll work alongside skilled nursing professionals, therapy staff, and physicians to develop personalized treatment plans that improve quality of life for our residents. Responsibilities Evaluate residents and develop individualized physical therapy treatment plans. Provide evidence-based therapeutic interventions to improve mobility, balance, strength, endurance, and functional independence. Monitor patient progress and modify treatment plans as appropriate. Collaborate with physicians, nursing staff, families, and the rehabilitation team to achieve optimal patient outcomes. Complete timely and accurate documentation in accordance with company policies and regulatory standards. Educate residents, caregivers, and staff on therapeutic exercises and safety techniques. Maintain compliance with state, federal, and professional practice standards. Qualifications Graduate of an accredited Physical Therapy program. Current Texas Physical Therapist license in good standing. CPR certification preferred. Experience in skilled nursing or long-term care is preferred but not required.
