Foursquare Healthcare is seeking a dedicated, patient-centered Full-Time Physical Therapist (PT) to join our clinical team. This position offers a unique dual-facility role, delivering full-time physical therapy services split between two of our premier skilled nursing and rehabilitation facilities: Whitney Nursing and Rehabilitation and Hillside Medical Lodge . Compensation, Work Schedule & Benefits Competitive Pay:

$44 - $54 per hour starting pay, with rates increasing based on years of relevant experience.

Schedule:

Full-time hours. Shifts will be consistently split between both sites based on facility needs and clinical coverage.

Responsibilities Perform initial physical therapy evaluations, establish functional goals, and implement evidence-based plan-of-care interventions. Manage a full-time clinical caseload with shifts split between Whitney Nursing and Rehabilitation and Hillside Medical Lodge , ensuring high standards of care across both locations. Work closely with interdisciplinary care teams—including physicians, nursing staff, occupational therapists, and speech-language pathologists—to optimize resident outcomes. Regularly assess patient progress, updating treatment plans and discharge strategies in accordance with facility policies and regulatory standards. Maintain timely and accurate clinical documentation using electronic medical records (EMR) systems. Educate residents, family members, and nursing staff on safe mobility techniques, fall prevention strategies, and home exercise routines.

Qualifications & Requirements Education:

Graduate of an accredited Physical Therapy program (Doctorate, Master's, or Bachelor's degree in Physical Therapy).

Licensure:

Active, unencumbered state Physical Therapy (PT) license.

Experience:

Prior experience in a skilled nursing facility (SNF), long-term care, or sub-acute rehabilitation preferred (strong new graduates will be considered).

Mobility & Travel:

Valid driver's license, reliable transportation, and willingness to commute between Whitney Nursing and Rehabilitation and Hillside Medical Lodge for scheduled shifts. (

This position involves travel between facilities, but mileage reimbursement is not provided).

Certifications:

Active

CPR / BLS

certification.
