Work Environment The position is primarily onsite, providing care directly in patients' homes. Key Responsibilities Perform thorough evaluations of patients to identify functional limitations and disabilities, developing individualized treatment plans based on assessment findings. Deliver evidence-based therapeutic interventions, ensuring that all treatments are documented accurately, including daily notes, evaluations, and discharge summaries. Consult with other healthcare providers regarding patient care continuity, ensuring proper communication and collaboration for optimal outcomes. Educate patients and their families about treatment protocols, progress, and post-discharge plans, empowering them to take an active role in their recovery. Maintain a high standard of customer service in interactions, respecting cultural differences and addressing the unique needs of a diverse patient population. Supervise and mentor support staff, providing guidance on patient care and departmental operations. Patient Population / Client Focus Your primary focus will be on patients with orthopedic conditions, neurological impairments, and post-surgical rehabilitation needs. The position will require you to adapt treatment plans to cater to a wide array of ages and backgrounds, ensuring each patient's unique needs are met effectively. Qualifications Minimum of a Bachelor's Degree in Physical Therapy from an accredited institution. Active State License in Physical Therapy is required for practice. Basic Life Support (BLS) certification must be obtained prior to starting the role. Strong written and verbal communication skills to facilitate effective patient education and interdisciplinary collaboration. Proficiency in using documentation software and basic computer applications is essential. Preferred Experience A minimum of two years of clinical experience in physical therapy is preferred, especially in home care or outpatient settings, where independent judgment and strong interpersonal skills are crucial. Familiarity with community resources and prior experience working with diverse patient populations will be advantageous. Compensation and Benefits The salary for this role ranges from $100,000 to $120,000 per year, commensurate with experience and qualifications.
