Physical Therapist (PT) - Full-Time Autumn Lake at Nellas | Elkins, WV $43-$50/hour | Full-Time | Benefits Ultimate Therapy is seeking a Physical Therapist (PT) to join the rehabilitation team at Autumn Lake at Nellas. This full-time opportunity offers the chance to provide individualized care while helping residents improve mobility, rebuild strength, and work toward greater functional independence. What You'll Do Complete physical therapy evaluations and develop individualized plans of care Provide skilled treatment focused on strength, gait, balance, transfers, endurance, and functional mobility Establish measurable rehabilitation goals and track resident progress Adjust treatment approaches based on clinical findings and response to therapy Educate residents and caregivers on exercises, mobility techniques, and safety strategies Maintain accurate and timely clinical documentation Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary care team What We Offer $43-$50/hour Full-time position Medical, dental, and vision insurance 401(k) Paid time off and holiday pay Continuing education support Collaborative rehabilitation team Short-term rehabilitation and long-term care caseload Opportunities for professional development and growth Qualifications Active West Virginia Physical Therapist license required Graduate of an accredited Physical Therapy program Strong clinical judgment, communication, and teamwork skills Skilled nursing or rehabilitation experience preferred, but not required New graduates are welcome to apply If you're a Physical Therapist looking to help residents achieve meaningful progress while working alongside a supportive rehabilitation team, we'd love to hear from you.
