Alliance Home Care is a fully licensed and insured skilled home health care agency providing services to families in the West Valley and Phoenix area. We are seeking a skilled and compassionate Physical Therapist to join our team. As a Physical Therapist, you will work with patients to help them regain movement and manage pain after an injury or illness in their own homes. Responsibilities Conduct thorough evaluations of patients' conditions and develop personalized treatment plans Implement treatment plans using a variety of techniques including exercise, manual therapy, and modalities Monitor patients' progress and adjust treatment plans as needed Educate patients and their families on exercises and techniques for at-home care Collaborate with other healthcare professionals to ensure comprehensive patient care Maintain accurate and up-to-date patient records Qualifications One, or more, years in Home Health Agency or in a hospital setting is preferred but not required Must have criminal background check Must have current CPR certification. Must have the ability to drive and the ability to access patients' homes.
