Job Description

South Pacific Rehab Services is seeking a compassionate Physical Therapist Assistant to provide high-quality rehabilitative care in skilled nursing and post-acute settings. Working under the direction of a Physical Therapist, you'll implement individualized treatment plans, assist with gait and balance training, therapeutic exercises, and functional mobility to help residents regain independence and quality of life. Responsibilities Implement physical therapy treatment plans under the supervision of a licensed Physical Therapist Provide therapeutic exercises, gait training, balance and transfer training to residents Assist residents with functional mobility and activities of daily living to improve independence Monitor, document, and report patient progress, response to treatment, and functional outcomes Educate patients, families, and caregivers on exercise programs, safety, and use of assistive devices Collaborate with PTs, nursing, and interdisciplinary team members to coordinate resident care Ensure accurate, timely, and compliant clinical documentation in the medical record Maintain equipment, treatment areas, and infection control standards in accordance with facility policies Adhere to ethical, regulatory, and professional practice standards for PTAs Participate in in-service training, continuing education, and quality improvement initiatives Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility and transfer training Skilled nursing/post-acute rehab experience Patient and caregiver education Clinical documentation and EMR use Infection control and safety practices Use of assistive devices and adaptive equipment Interdisciplinary team collaboration Knowledge of rehabilitation principles and functional outcomes measurement
