Job Description
South Pacific Rehab Services is seeking a dedicated Physical Therapist Assistant to join our Healthcare & Medical Services team. In this role, you will provide direct patient care under the supervision of a Physical Therapist in skilled nursing and post-acute settings. Responsibilities include implementing treatment plans, documenting progress, and collaborating with an interdisciplinary team to promote functional recovery and quality of life. Responsibilities Deliver skilled PTA interventions under the direction of a Physical Therapist for residents in skilled nursing and post-acute settings. Implement individualized treatment plans focused on functional mobility, strength, balance, and safety. Accurately document treatments, progress notes, and outcomes in the EMR in a timely, compliant manner. Monitor resident response to therapy, report changes to the PT and care team, and adjust interventions within scope. Educate residents and caregivers on exercises, mobility strategies, and safe use of assistive devices. Collaborate with nursing, rehab, and interdisciplinary teams to support coordinated, patient-first care. Participate in care conferences, discharge planning, and fall-prevention or mobility-focused initiatives. Adhere to ethical, evidence-based practice standards and all regulatory and facility policies. Engage in ongoing education and competency development supported by clinical leadership. Contribute to a clinician-centered, respectful work culture that prioritizes communication and quality outcomes. Required Skills Therapeutic exercise implementation Gait training and balance interventions Functional mobility training (transfers, bed mobility) Use of assistive devices and DMETherapy documentation in EMR systems Vital signs monitoring and patient safety Knowledge of post-acute and long-term care rehab Care planning and goal progression with PTPatient and family education Interdisciplinary team collaboration
