Licensed Physical Therapist Assistant Prava Therapy - 4.2 Sierra Madre, CA Job Details Full-time $33 - $38 an hour 1 day ago Qualifications Licensed Physical Therapist Assistant Full Job Description
FULL-TIME PHYSICAL THERAPIST ASSISTANT
The Kensington Sierra Madre | Sierra Madre, California Prava Therapy What if the best part of your therapy day wasn't the treatment session? It's watching a resident take that extra step. Seeing someone stand up with more confidence. Walking with a resident to dinner and realizing they're moving differently than they were last week. Hearing, "I couldn't do that before." Those moments are why great PTAs do what they do. Prava Therapy is looking for a Full-Time Physical Therapist Assistant (PTA) to join our team at The Kensington Sierra Madre. If you want to practice therapy in a place where you can know your residents, build relationships, and see your work translate into everyday life , this could be your next great opportunity.
YOUR TREATMENT PLAN DOESN'T END WHEN THE SESSION DOES.
In senior living, you get something that traditional outpatient settings don't always offer: You get to see the whole picture. You see how a resident moves through their day. You see what happens in the hallway. You notice when getting out of a chair suddenly takes more effort. You see whether the strategies you're teaching actually carry over into real life. And you can turn those observations into better care. At Prava, we want our PTAs to be clinicians—not just implementers. Bring your clinical judgment. Bring your curiosity. Bring your ideas.
WHAT YOU'LL DO
Working under the direction of a Physical Therapist, you'll deliver skilled interventions that help residents move with greater safety, confidence, and independence.
You'll:
Provide skilled physical therapy interventions in accordance with the established plan of care. Help residents improve strength, balance, gait, transfers, endurance, mobility, and functional performance. Use meaningful, functional activities to help residents achieve their goals. Incorporate the resident's actual environment and daily routines into treatment whenever appropriate. Recognize changes in mobility or functional status and communicate them promptly to the supervising PT and interdisciplinary team. Help residents work toward the things they actually want to do—walking to meals, participating in activities, navigating their apartment, or staying active in their community. Provide education and reinforcement to residents, caregivers, and community staff. Collaborate with PTs, OTs, nursing, caregivers, families, and community leadership. Build strong relationships with residents and families. Document efficiently and thoughtfully using Prava's technology and AI-enabled tools. Participate in a therapy culture focused on clinical excellence, teamwork, accountability, and resident experience. Help create a therapy presence that is visible and integrated throughout the community.
WE BELIEVE
PTAs
ARE CLINICIANS.
You're not here simply to count repetitions. You're here to understand why you're doing them. Why does this resident need more strength? Why is their balance changing? What is keeping them from walking to the dining room? What would make them feel more confident? What can we change in the environment? What can we practice differently? At Prava, we believe: Every intervention should have a functional purpose. Residents should be appropriately challenged—not underestimated. The environment can be part of the solution. Caregiver education is part of treatment. Outcomes matter more than minutes. Dignity and autonomy are clinical priorities.
YOU'LL HAVE A COMMUNITY
—
NOT JUST A CASELOAD.
One of the best things about senior living therapy is the relationship you get to build. You don't just see a resident for an hour and disappear. You become familiar. You become trusted. You might see them in the dining room after your session. You might hear about their family visiting. You might notice they're having a harder day before anyone else does. That connection matters. Because when residents know you, therapy becomes a partnership.
PRAVA GIVES YOU THE TOOLS TO DO YOUR BEST WORK.
You bring the clinical expertise. We bring the operating system. Prava combines modern technology, AI-enabled documentation, clinical standards, operational support, credentialing, billing and revenue cycle infrastructure, and a team committed to helping clinicians succeed. We want technology to remove friction from your day—not create more of it. So you can spend more time doing what you do best: Helping people move. Helping people participate. Helping people live.
THIS ROLE IS FOR YOU
IF... You're a PTA who takes pride in doing excellent clinical work. You genuinely enjoy working with older adults. You believe movement is about more than strength and range of motion. You like getting to know your residents. You're confident working independently while knowing when to collaborate or ask for support. You notice the little things. You're naturally encouraging and know how to motivate without pushing too hard. You enjoy working as part of an interdisciplinary team. You want your therapy to carry over into real life. You want to practice somewhere that values clinical judgment, relationships, and outcomes—not just visit volume.
WHAT SUCCESS LOOKS LIKE A
resident gets up more confidently. A caregiver knows exactly how to support a transfer. A resident who stopped walking to activities starts going again. Someone who was afraid of falling feels confident enough to move through their day. And you get to see those changes happen—not just read about them in a progress note. That's the reward.
COME PRACTICE PT DIFFERENTLY.
If you believe physical therapy should help people do more than move better— we think you'll fit right in. Bring your clinical expertise. Bring your energy. Bring your curiosity. We'll bring the operating system. Come help residents keep moving, keep participating, and keep living.
