Job Description

South Pacific Rehab Services is seeking a Certified Physical Therapist Assistant to provide resident-centered care in skilled nursing and post-acute settings. Under the supervision of a licensed PT, you will deliver evidence-based interventions to improve strength, mobility, balance, and safety, supporting residents in reaching their highest functional potential. Responsibilities include implementing treatment plans, documenting in the EHR, participating in care planning, and collaborating with an interdisciplinary team. You'll work in a clinician-centered culture that values ethical practice, open communication, and continuous learning, with access to strong clinical support, ongoing education, and growth in a stable, mission-driven rehab organization. Responsibilities Deliver physical therapy interventions under the direction of a licensed PT in SNF and post-acute settings Implement individualized treatment plans focused on functional mobility, safety, and quality of life Accurately document treatments, progress notes, and billing in the electronic health record Assist with resident assessments, outcome tracking, and care plan updates Educate residents and families on exercises, mobility strategies, and safe transfers Collaborate closely with PTs, OTs, SLPs, nursing, and facility staff to coordinate resident-centered care Adhere to all regulatory, ethical, and company clinical standards Participate in in-service training, continuing education, and performance improvement initiatives Required Skills Therapeutic exercise implementation Gait training and balance interventions Functional mobility and transfer training Safe patient handling and body mechanics Use of rehab equipment and modalities Electronic health record documentation Vital signs monitoring and patient observation Care planning in SNF/post-acute settings Patient and family education Interdisciplinary team collaboration
