Job Description

Tender Touch Rehab Services is seeking a dedicated Physical Therapist Assistant to join our Healthcare & Medical Services team. In this role, you will provide high-quality therapy services to seniors in skilled nursing and long-term care settings, implementing treatment plans under the direction of a Physical Therapist. You will help improve residents' function, independence, and quality of life while collaborating with an interdisciplinary team. Responsibilities Provide physical therapy interventions under the direction of a licensed Physical Therapist for seniors in skilled nursing and long-term care settings. Implement individualized treatment plans focused on improving mobility, function, independence, and safety. Document patient progress, outcomes, and treatment in accordance with regulatory and company standards. Collaborate with PTs, nurses, and the interdisciplinary team to adjust care plans and support discharge planning. Educate patients, families, and caregivers on exercises, safety, and functional strategies. Maintain compliance with all facility, state, and federal regulations and uphold high clinical quality standards. Contribute to a supportive, patient-centered culture that promotes dignity, compassion, and respect. Required Skills Therapeutic exercise implementation Gait and balance training Transfer and mobility training Use of physical therapy modalities Patient assessment and documentation (EMR) Care planning and goal setting Knowledge of geriatric conditions and fall prevention Infection control and safety compliance Interdisciplinary team collaboration Patient and family education
