Job Description
Tender Touch Rehab Services is seeking a compassionate Physical Therapist Assistant to provide high-quality, evidence-based therapy for seniors in skilled nursing and long-term care settings. The PTA will implement individualized treatment plans under the direction of a Physical Therapist, focusing on improving mobility, function, and independence. Responsibilities include conducting therapeutic exercises, gait and balance training, patient and family education, and accurate documentation. Responsibilities Implement PT treatment plans for senior patients under direction of a licensed Physical Therapist Provide therapeutic exercises, gait training, balance and transfer training to improve mobility and function Assist with pain management strategies and use of therapeutic modalities as permitted by state practice act Educate patients, families, and caregivers on home exercise programs, safety, and use of assistive devices Accurately document treatments, patient progress, and outcomes in the electronic medical record Collaborate with PTs, OTs, SLPs, nursing, and medical staff as part of an interdisciplinary care team Monitor patient response to treatment and report changes in condition to the Physical Therapist and nursing staff Assist in discharge planning and recommendations for equipment and continued services Ensure compliance with federal, state, and facility regulations, as well as payer and documentation requirements Contribute to a supportive, patient-centered environment that promotes dignity, independence, and quality of life for seniors Required Skills Therapeutic exercise implementation Gait and balance training Transfer and mobility training Geriatric rehabilitation Skilled nursing/long-term care rehab Patient and family education Therapeutic modalities (per state practice) Functional outcome measurement and documentation Electronic medical records (EMR) documentation Care planning within an interdisciplinary team
