Tender Touch Rehab Services seeks a compassionate Physical Therapist Assistant to provide high-quality rehabilitative care for seniors in skilled nursing and long-term care settings. Working under the direction of a Physical Therapist, you will implement individualized treatment plans, assist with therapeutic exercises and mobility training, document progress, and educate patients and families. You'll collaborate closely with an interdisciplinary team in a supportive, patient-centered culture that values dignity, respect, and clinical excellence, with opportunities for mentorship, professional growth, and advancement in diverse post-acute environments. Responsibilities Implement individualized treatment plans under direction of a Physical Therapist Assist patients with therapeutic exercises, gait, balance, and transfer training Monitor, document, and report patient progress and response to treatment Educate patients, families, and caregivers on exercises and safety techniques Collaborate with an interdisciplinary team to optimize functional outcomes Ensure accurate, timely, and compliant clinical documentation Maintain a safe, clean, and supportive therapy environment Participate in care planning, case conferences, and discharge planning Support evidence-based, patient-centered practice and clinical excellence Contribute to a culture of dignity, compassion, and respect for seniors Required Skills Therapeutic exercise implementation Gait and balance training Transfer and mobility training Geriatric rehabilitation Skilled nursing/long-term care rehab Clinical documentation (EMR) Patient and family education Functional outcome measurement Infection control and safety practices Interdisciplinary team collaboration
