Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support high-quality therapy services for seniors in skilled nursing and post-acute care. In this role, you will implement PT treatment plans focused on restoring mobility, function, and independence, while delivering patient-centered, evidence-based care. You'll collaborate closely with supervising PTs and an interdisciplinary team, document in EMR, and educate patients and families. Responsibilities Implement PT treatment plans for senior patients in skilled nursing and post-acute settings Provide therapeutic exercises, gait training, and functional mobility interventions Monitor patient progress and communicate changes to supervising Physical Therapist Accurately document treatments, outcomes, and patient responses in the EMR system Educate patients, families, and caregivers on exercises, safety, and functional goals Collaborate with an interdisciplinary team to support safe, person-centered care plans Assist with discharge planning and recommendations for continued therapy or equipment needs Uphold company values of integrity, respect, and quality outcomes in all patient interactions Required Skills Therapeutic exercise implementation Gait training and balance interventions Functional mobility training Therapeutic modalities (heat, cold, e-stim as permitted) Patient transfer and safe lifting techniques Documentation in EMR systems Care planning in skilled nursing/post-acute settings Patient and family education ROM and strength assessment within PTA scope Interdisciplinary team collaboration
