Job Description

Renewal Rehab seeks a compassionate Physical Therapist Assistant to support seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will implement treatment plans, provide therapeutic exercises and functional training, document progress, and educate patients and families. You'll collaborate closely with an interdisciplinary team in a patient-centered, integrity-driven culture that emphasizes respect, quality outcomes, and open communication. Responsibilities Deliver therapeutic exercises, gait training, and functional mobility interventions under PT direction Assist in implementing and progressing individualized treatment plans for senior patients Accurately document treatments, patient responses, and progress in the medical record Educate patients, families, and caregivers on safe mobility and home exercise programs Collaborate with PTs, nurses, and interdisciplinary team to support discharge planning and care goals Ensure compliance with state, federal, and facility regulations and company clinical standards Maintain a safe, clean treatment environment and proper use of equipment Participate in case conferences, in-services, and continuing education to enhance clinical skills Support a patient-centered, respectful culture focused on quality outcomes for seniors Contribute to productivity expectations while maintaining compassionate, ethical care Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility and transfer training Geriatric rehabilitation techniques Skilled nursing and post-acute care documentation Patient and family education Use of rehab equipment and assistive devices Electronic medical record (EMR) proficiency Knowledge of Medicare and rehabilitation regulations Interdisciplinary team collaboration
