Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support seniors in skilled nursing and post-acute care settings. In this on-site role, you will deliver evidence-based interventions under the direction of a Physical Therapist to improve mobility, strength, balance, and functional independence. You will collaborate closely with an interdisciplinary team, maintain accurate documentation, and educate patients and families. Responsibilities Deliver one-on-one physical therapy interventions to seniors under the direction of a Physical Therapist Implement individualized treatment plans to improve mobility, strength, balance, and functional independence Accurately document treatments, patient progress, and outcomes in the EMR in a timely manner Collaborate with PTs, nursing, and interdisciplinary team members to support coordinated patient care Educate patients and families on home exercises, safety, and strategies to maintain functional gains Assist with caseload management to ensure efficient, patient-centered care in skilled nursing and post-acute settings Contribute to a culture of integrity, respect, and clinical excellence in senior care Participate in in-service training, mentorship, and professional development opportunities Required Skills Therapeutic exercise implementation Gait training and balance interventions Neuromuscular re-education Functional mobility and transfer training Use of assistive devices and DMEPatient observation and clinical documentation Electronic medical record (EMR) use Treatment progression under PT plan of care Fall prevention techniques Patient and family education
