Renewal Rehab seeks a compassionate Physical Therapist Assistant to provide high-quality rehabilitative care to seniors in skilled nursing and post acute settings. Working under the direction of a Physical Therapist, you will implement treatment plans, lead therapeutic exercises, monitor patient progress, and document outcomes in accordance with clinical standards. You'll collaborate with an interdisciplinary team, support safe functional mobility, and educate patients and caregivers. Responsibilities Implement PT treatment plans under supervision of a licensed Physical Therapist Provide therapeutic exercises, gait training, balance and functional mobility interventions for seniors Assist with transfer training, ADL-related mobility, and fall prevention strategies Accurately document treatments, patient progress, and outcomes in EMR per regulatory standards Communicate patient status and recommendations to the PT and interdisciplinary care team Educate patients, families, and caregivers on home exercise programs and safety techniques Participate in care planning, team meetings, and discharge planning as needed Ensure compliance with state, federal, and facility regulations and PT practice standards Maintain a safe, clean, and patient-centered therapy environment Contribute to quality improvement initiatives and support a culture of clinical excellence Required Skills Physical therapy interventions for geriatrics Therapeutic exercise and functional mobility training Gait and balance training Transfer training and safe patient handling Use of assistive devices and DMETreatment documentation and EMR proficiency Understanding of PT/PTA scope of practice and regulations Care planning in skilled nursing/post-acute settings Patient and caregiver education Interdisciplinary team collaboration
