We are seeking a dedicated and energetic Licensed Physical Therapy Assistant (LPTA) to join our dynamic healthcare team . In this vital role, you will work closely with licensed physical therapists to deliver exceptional patient care inside Assistive Living Facilities. We have positions available in Reed City and Big Rapids Your expertise will help patients regain mobility, reduce pain, and improve their overall quality of life through tailored physiotherapy interventions. This position offers a rewarding opportunity to make a meaningful difference in patients' recovery journeys while expanding your professional skills in diverse clinical contexts. Responsibilities Assist licensed physical therapists in developing and implementing individualized care plans for primarily geriatric patients. Conduct therapeutic exercises, manual therapy techniques such as trigger point therapy, and modalities under supervision to promote healing and mobility. Monitor patient progress, document responses to treatment, and communicate updates to the supervising therapist to adjust care plans accordingly. Support discharge planning by preparing patients for discharge from therapy services by providing staff education and training and assist in developing Home Exercise Program ensuring they understand prescribed exercises and precautions. Maintain accurate medical terminology records, treatment logs, and progress notes in compliance with healthcare regulations. Educate patients on physiotherapy techniques, injury prevention strategies, and self-care practices to foster independence. Collaborate with multidisciplinary teams in inpatient, outpatient, orthopedic, and specialized clinics to ensure comprehensive patient care. Skills Proficiency in physiotherapy techniques including trigger point therapy and other manual therapies. Strong knowledge of anatomy and physiology relevant to musculoskeletal, neurological, and pediatric conditions. Experience working in Assistive Living Facilities, skilled nursing facilities, home health settings, outpatient clinics or acute care environments is not required but helpful. Familiarity with medical terminology and documentation standards necessary for accurate record keeping. Ability to develop and follow detailed care plans tailored to individual patient needs. Excellent patient care skills with a compassionate approach that encourages patient engagement and adherence. Knowledge of discharge planning processes to facilitate seamless transitions from clinical settings to home or community-based care.
