Job Description

Renewal Rehab seeks a compassionate Physical Therapist Assistant to support high-quality rehab services for seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will implement individualized treatment plans, guide therapeutic exercises and functional mobility training, and document patient progress. You'll collaborate with an interdisciplinary team, educate patients and families, and help promote safety and independence. Responsibilities Implement PT treatment plans under supervision of a licensed Physical Therapist Provide therapeutic exercises, gait training, and functional mobility interventions Assist with balance, strength, and endurance programs for senior patients Monitor, document, and report patient progress and response to treatment Educate patients, families, and caregivers on home exercise and safety strategies Collaborate with an interdisciplinary team to optimize functional outcomes Ensure compliance with state, federal, and facility regulations and documentation standards Maintain accurate, timely electronic medical records Contribute to discharge planning and recommendations for ongoing services or equipment Promote a safe, respectful, and patient-centered therapy environment Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Geriatric rehabilitation Skilled nursing/post-acute care documentation Use of therapy modalities (e.g., heat, cold, e-stim as allowed by state) Patient and family education Electronic medical record (EMR) proficiency Safety and fall prevention strategies Interdisciplinary care collaboration
