Job Description

Renewal Rehab seeks a compassionate Physical Therapist Assistant to provide hands-on rehabilitative care for seniors in post-acute and long-term care settings. Working under the supervision of a licensed Physical Therapist, you will deliver therapeutic exercises, gait and balance training, and functional interventions to improve mobility, safety, and quality of life. You'll collaborate closely with an interdisciplinary team, document progress in EMR, and educate patients and families. Responsibilities Provide direct treatment to seniors under the direction of a licensed PT to improve mobility, strength, and function. Assist in developing and progressing individualized treatment plans aligned with patient goals and clinical best practices. Accurately document treatments, patient progress, and outcomes in the electronic medical record in a timely manner. Collaborate with PTs, nursing, and interdisciplinary teams to support safe discharges and continuity of care. Educate patients and families on home exercises, safety strategies, and functional techniques to maximize independence. Adhere to all regulatory, ethical, and company standards, promoting a culture of integrity, respect, and quality outcomes. Required Skills Therapeutic exercise implementation Gait training and balance interventions Manual therapy techniques within PTA scope Use of rehab equipment and modalities Patient transfer and safe mobility techniques Documentation in EMR/rehab software Care planning with interdisciplinary teams Geriatric rehabilitation principles Patient and family education Infection control and safety compliance
