Job Description

Renewal Rehab seeks a compassionate Physical Therapist Assistant to provide evidence-based therapy services for seniors in skilled nursing and post-acute settings. Working under the direction of a Physical Therapist, you'll deliver individual and group treatments, assist with evaluations and care plans, and document services in accordance with regulatory standards. You'll collaborate with an interdisciplinary team to promote mobility, safety, and functional independence, supported by strong clinical resources, mentorship, and a patient-centered culture focused on quality outcomes and integrity. Responsibilities Implement physical therapy treatment plans under supervision of a licensed Physical Therapist Provide one-on-one and group therapy interventions to geriatric patients in SNF/post-acute settings Assist with patient assessments, goal setting, and progress updates Accurately document treatments and outcomes in compliance with Medicare and facility guidelines Educate patients, families, and caregivers on mobility, safety, and home exercise programs Collaborate with nursing, rehab, and medical staff to optimize functional outcomes Monitor patient response, adjust interventions per PT direction, and report changes in condition Maintain equipment and treatment areas in a clean, safe, and organized manner Participate in care conferences, in-services, and clinical education activities Uphold ethical, patient-centered care aligned with Renewal Rehab's values of integrity and respect Required Skills Therapeutic exercise and functional mobility training Gait training and transfer techniques Geriatric rehabilitation principles Balance, fall prevention, and safety training Use of assistive devices and DME fitting Skilled documentation and EMR proficiency Knowledge of Medicare and reimbursement guidelines Therapeutic modalities (e.g., heat, cold, e-stim, ultrasound) per state practice act Patient and caregiver education Interdisciplinary team collaboration and communication
