Job Description
Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support seniors in skilled nursing and post-acute care. In this on-site role within our Healthcare & Medical Services team, you will carry out individualized treatment plans designed by a Physical Therapist to improve mobility, strength, and functional independence. You'll document care accurately, collaborate with an interdisciplinary team, and educate patients and families. Responsibilities Deliver one-on-one physical therapy interventions to seniors under the direction of a Physical Therapist. Implement individualized treatment plans focused on mobility, strength, balance, and functional independence. Accurately document treatments, progress, and outcomes in the EMR while meeting regulatory standards. Assist with patient assessments and communicate changes in condition to the supervising PT and care team. Educate patients, families, and caregivers on exercises, safety, and strategies to maintain function. Collaborate with nurses, physicians, and other therapists to coordinate patient-centered care plans. Support safe discharge planning by contributing to functional status and equipment recommendations. Required Skills Therapeutic exercise implementation Gait training and balance interventions Functional mobility training (transfers, bed mobility) Therapeutic modalities application (heat, cold, e-stim, etc.) Patient treatment documentation in EMR systems Care planning in skilled nursing/post-acute settings Patient and family education on home programs Vital signs monitoring and patient safety Interdisciplinary team collaboration Knowledge of Medicare and rehab documentation standards
