Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support physical therapy services for seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you'll assist with treatment sessions, functional mobility training, and restorative exercise programs to improve strength, balance, and independence. You'll document patient progress, collaborate within an interdisciplinary care team, and educate patients and families. Responsibilities Implement treatment plans under supervision of a Physical Therapist Provide therapeutic exercises, gait and balance training, and functional mobility activities Assist with transfer training and use of assistive devices safely Observe, measure, and document patient progress and response to treatment Communicate effectively with PTs, nursing, and interdisciplinary team members Educate patients and families on exercises, safety, and functional goals Ensure adherence to regulatory, documentation, and reimbursement requirements Maintain a safe, clean, and patient-centered treatment environment Contribute to discharge planning and recommendations for continued services Participate in clinical meetings, in-services, and professional development Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility and transfer training Knowledge of geriatric rehabilitation Skilled nursing/post-acute documentation Use of assistive devices and DMEPatient and family education Electronic medical record (EMR) documentation Understanding of Medicare and rehab regulations Interdisciplinary team collaboration
