Under the direction of the supervising Physical Therapist, you'll provide one-on-one treatment designed around what each patient needs to do in their actual life . For some patients, that's recovering after surgery or hospitalization. For others, it's rebuilding strength after an illness, reducing falls, managing a neurological condition, or fighting against a gradual loss of mobility. One Home. Goals That Actually Matter to Them. You'll work with patients experiencing orthopedic, neurological, post-surgical, post-hospitalization, balance, strength, and mobility challenges. Your supervising PT establishes the plan of care. You help turn that plan into everyday function. That could mean working repeatedly on transfers until your patient can safely get out of bed. Building lower-extremity strength so they can manage their stairs. Practicing gait until they can confidently move through their home. Teaching a caregiver how to safely assist without doing everything for the patient. Or helping someone rebuild enough endurance to participate in the parts of life they've been missing. What You'll Be Doing Provide skilled Physical Therapy treatment in patient homes Implement individualized plans of care established by the supervising PT Provide strengthening, gait, balance, transfer, mobility, and functional training Reinforce individualized home exercise programs Educate patients and caregivers on safe mobility and functional techniques Monitor patient response and progress Recognize and communicate meaningful changes to the supervising PT Support fall prevention and home safety goals Complete accurate and timely EMR documentation Collaborate with PTs, OTs, nurses, and other interdisciplinary clinicians Your Rehab Experience Can Come With You You don't need to have spent your entire career in home health. Experience in Outpatient Therapy, Skilled Nursing, Inpatient Rehabilitation, Transitional Care, Orthopedics, Senior Living, Long-Term Care, Hospital Rehab, and other rehabilitation environments can translate well into this role. We're looking for a PTA who knows how to connect with patients, provide excellent hands-on treatment, recognize changes, communicate with the supervising PT, and stay organized while working independently in the field. What You Bring Active Minnesota Physical Therapist Assistant (PTA) license Valid driver's license, reliable transportation, and automobile insurance Strong hands-on treatment skills Excellent patient and caregiver communication Ability to independently manage a field-based schedule Strong organization and documentation skills Commitment to patient-centered rehabilitation Home health experience and experience with geriatric or medically complex patients are preferred, but not required.
