Job Description

Oasis Therapy Services is committed to providing exceptional therapy services to seniors in a welcoming and supportive environment. Oasis Therapy Services is seeking a Team Lead Physical Therapist Assistant to provide one-on-one rehabilitation services and assist with therapy operations in independent living, assisted living, and memory care settings in Litchfield, Minnesota.
EMR:
Net Health Responsibilities Provide physical therapy interventions directed by and under the supervision of a licensed Physical Therapist. Carry out treatment plans focused on strength, balance, gait, mobility, safety, and independence. Monitor residents' progress and report changes or concerns to the supervising Physical Therapist. Participate in care conferences within the PTA's scope of practice. Serve as a liaison among residents, facility staff, and the therapy team. Assist the Director of Rehabilitation with assigned administrative and non-clinical managerial tasks. Complete accurate and timely Net Health documentation. Qualifications Active and current Minnesota Physical Therapist Assistant license. Graduate of a CAPTE-accredited Physical Therapist Assistant program. Current CPR certification. Strong leadership, communication, organization, and documentation skills. Geriatric, independent living, assisted living, memory care, or rehabilitation experience is preferred. Why Join Oasis?
