Your schedule is built around patient locations, geography, and current coverage needs , rather than expecting you to drive back and forth between every community in the service area each day. Where you live matters. If you're a PTA in or around St. Cloud, Sauk Centre, or a nearby Central Minnesota community , we'd like to talk about where you could fit within our coverage. Help Patients Keep Moving Forward You'll work closely with a supervising Physical Therapist to carry out individualized plans of care for patients recovering from hospitalization, surgery, illness, injury, falls, deconditioning, and other changes in mobility. Your visits may focus on strengthening, gait and balance training, transfers, mobility, functional activities, and home exercise programs. You'll also monitor how patients are progressing, communicate meaningful changes to the supervising PT, and provide the encouragement and consistency that helps patients stay engaged in their recovery. What You'll Be Doing Provide skilled Physical Therapy treatment in patient homes Implement individualized plans of care developed by the supervising PT Provide strengthening, gait, balance, ROM, transfer, and mobility training Reinforce individualized home exercise programs Educate patients and caregivers on mobility and safety Monitor patient response and progress Communicate clinical observations and changes to the supervising Physical Therapist Complete accurate and timely EMR documentation Collaborate with PTs, nurses, OTs, and other interdisciplinary clinicians Bring the PTA Experience You Already Have You don't need to come from home health to understand how to connect with a patient. We're interested in PTAs with backgrounds in Outpatient Therapy, Orthopedics, Skilled Nursing, Inpatient Rehabilitation, Transitional Care, Long-Term Care, Senior Living, Home Health, and other rehabilitation settings. We're looking for someone who combines strong hands-on treatment skills with the ability to build trust, motivate patients, stay organized, and work independently in the field. What You Bring Active Minnesota Physical Therapist Assistant license Valid driver's license, automobile insurance, and reliable transportation Strong hands-on treatment skills Excellent patient and caregiver communication Ability to independently manage a field-based schedule Strong collaboration with supervising Physical Therapists Commitment to patient-centered rehabilitation Home health experience preferred Experience with geriatric or medically complex patients preferred Be More Than the Person Who Runs Through the Exercises The best PTAs know how to push when a patient needs a challenge, encourage when they're frustrated, recognize when something has changed, and celebrate the progress that keeps them moving forward.
