In home health, you're already there. You're helping a patient practice the actual stairs they need to climb every night. You're strengthening the muscles they need to safely get out of their own bed. You're working on balance in the environment where they're most concerned about falling. You're teaching caregivers strategies they can use immediately after you leave. There's no simulation required. The patient's everyday life becomes the treatment environment. See the Results of Your Work Differently As a PTA, you'll work under the direction of an experienced Physical Therapist while having significant independence throughout your workday. You'll provide treatment to patients recovering from surgery, hospitalization, illness, injury, deconditioning, falls, and changes in mobility. And because you're seeing patients where they live, progress becomes incredibly tangible. A stronger transfer means someone can use the bathroom independently. Better balance means they're less afraid to walk through their home. Improved endurance might mean they can finally get outside again. Those are the outcomes you'll help create. What You'll Be Doing Provide Physical Therapy treatment visits in patient homes. Implement individualized plans of care developed by the supervising PT. Provide strengthening, gait training, balance activities, ROM, mobility training, and functional exercises. Reinforce home exercise programs. Educate patients and caregivers on mobility, fall prevention, and safety. Monitor patient progress and response to treatment. Communicate clinical observations and changes to the supervising Physical Therapist. Complete accurate and timely EMR documentation. Collaborate with PTs, nurses, OTs, and other interdisciplinary clinicians. Bring the PTA Experience You Already Have You don't necessarily need previous home health experience to make the transition. We're interested in PTAs coming from Outpatient Therapy, Orthopedics, Skilled Nursing, Inpatient Rehabilitation, Transitional Care, Long-Term Care, Senior Living, and Home Health. What matters is your ability to connect with patients, work independently, stay organized, and deliver excellent hands-on care. Required Qualifications Active Minnesota Physical Therapist Assistant license Valid driver's license, automobile insurance, and reliable transportation Strong patient communication and relationship-building skills Ability to independently manage a field-based workday Strong collaboration with supervising Physical Therapists Commitment to patient-centered care Preferred Qualifications Home health experience Experience with geriatric or medically complex patients Experience with post-acute rehabilitation Comfort working independently in patient homes Keep Doing the Work You Love.
