Liberty Health is seeking a Home Care Physical Therapy Assistant to support patients in their homes with evidence-based, person-centered rehabilitation. Working under the direction of a licensed Physical Therapist, you will implement treatment plans, assist with therapeutic exercises, functional mobility, balance and gait training, and use of adaptive equipment. You will document progress accurately, communicate with an interdisciplinary care team, and educate patients and caregivers.
