Job Description

Job Summary We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. In this role, you will be responsible for assessing, planning, and implementing individualized treatment plans to improve patient mobility, reduce pain, and promote overall well-being. Your expertise will help patients recover from injuries, surgeries, or manage chronic conditions across various settings including outpatient clinics, hospitals, and home health environments. You will collaborate closely with other healthcare professionals to deliver comprehensive care that enhances patient outcomes and quality of life. Responsibilities Conduct thorough patient evaluations to determine physical therapy needs, including assessments of mobility, strength, and functional abilities. Develop personalized treatment plans utilizing evidence-based practices tailored to each patient's condition and goals. Implement therapeutic interventions such as manual therapy, trigger point therapy, therapeutic exercises, and modalities to facilitate recovery. Monitor patient progress regularly and adjust treatment plans accordingly to ensure optimal results. Document all patient interactions accurately in electronic health records (EHR) systems in compliance with clinical documentation standards. Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate care and discharge planning. Educate patients and their families on injury prevention, home exercises, and community resources for ongoing support. Qualifications Valid licensure or registration as a licensed Physical Therapist in the respective jurisdiction. Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health settings. Strong knowledge of physiology, anatomy, orthopedics, neurology, geriatrics, pediatrics, and related medical terminology. Experience working with geriatric patients and familiarity with care plans specific to older adults. Proficiency in outpatient rehabilitation techniques including physiotherapy modalities and trigger point therapy. Ability to develop effective treatment plans based on comprehensive assessments and patient needs. Excellent client communication skills with a focus on patient care and education. Experience managing electronic health records (EHR) or electronic medical records (EMR) systems efficiently. Knowledge of discharge planning processes and working within acute care or hospital environments is highly desirable. Demonstrated ability to collaborate effectively with healthcare teams across various disciplines for holistic patient care. Pay:
$30.00 - $40.00 per hour
