What you'll do This Physical Therapist Assistant (PTA) position is for Home Health Services, providing rehabilitative services to patients in their homes under the supervision of a licensed Physical Therapist. This role supports the implementation of individualized treatment plans aimed at improving mobility, relieving pain, and enhancing physical function. The PTA works closely with patients, caregivers, and the healthcare team to ensure effective and compassionate care in a home setting. Duties Assist licensed physical therapists in implementing individualized
  • physical therapy treatment plans
  • for patients of all ages Perform
  • manual therapy
  • techniques and other therapeutic modalities Operate
  • medical equipment
  • safely and effectively Document patient progress accurately following
  • clinical assessment documentation
  • and
  • medical documentation standards
  • Optimize patient outcomes through effective communication about treatment strategies and patient responses. Educate patients and caregivers on exercises, mobility techniques, and injury prevention strategies to promote recovery outside clinical settings. Manage
  • electronic health records (EHR)
  • efficiently to ensure seamless documentation and information sharing across the care team.
Experience Prior experience in clinical assessment documentation, medical terminology, and operating medical equipment Experience working in diverse healthcare environments such as outpatient clinics, inpatient units, skilled nursing facilities, or home health is highly desirable Knowledge of specialized areas including orthopedics, geriatrics, pediatrics, and acute care enhances the ability to provide comprehensive patient care Proven ability to communicate effectively with patients, families, and interdisciplinary teams Experience with caregiver communication and managing treatment for various populations is an asset Company Overview CHP Home Care & Hospice is a regional leader in home health and hospice services in northwest and west central Ohio.
