Job Description

Tender Touch Rehab Services is seeking a compassionate Physical Therapist Assistant to provide high-quality rehabilitative care for seniors in skilled nursing and long-term care settings. The PTA will implement individualized treatment plans under the direction of a Physical Therapist, focusing on improving mobility, function, and independence. Responsibilities include carrying out therapeutic exercises, gait and balance training, functional mobility activities, and patient/family education, while accurately documenting services. Responsibilities Implement physical therapy treatment plans under supervision of a Physical Therapist Provide therapeutic exercises, gait, balance, and functional mobility training Assist with transfer training and use of assistive devices and durable medical equipment Monitor patient progress and communicate changes to supervising PT and care team Document treatments, patient responses, and outcomes accurately and timely in EMREducate patients, families, and caregivers on exercises, safety, and home programs Collaborate with nursing, physicians, and interdisciplinary team members on care plans Assist with discharge planning and recommendations for equipment and continued services Ensure compliance with federal, state, and facility regulations and documentation standards Contribute to a supportive, patient-centered environment that respects dignity and independence Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility and transfer training Use and fitting of assistive devices (walkers, canes, wheelchairs) Geriatric rehabilitation techniques Skilled nursing/long-term care documentation and EMR use Patient and family education Range of motion and strengthening interventions Safety assessment and fall prevention Interdisciplinary team collaboration
