Job Description

Tender Touch Rehab Services is seeking a compassionate Physical Therapist Assistant to provide high-quality, evidence-based care to seniors in skilled nursing and long-term care settings. Working closely with Physical Therapists, you will implement treatment plans, assist with therapeutic exercises, gait and balance training, and functional mobility to enhance residents' independence and quality of life. You'll document progress, collaborate with an interdisciplinary team, and educate patients and families-all within a supportive, patient-centered culture that values clinical excellence, mentorship, and professional growth. Responsibilities Implement PT treatment plans under supervision of a licensed Physical Therapist Provide therapeutic exercises, gait, balance, and transfer training to senior patients Assist with pain management, mobility, and fall-prevention interventions Monitor, document, and report patient progress and response to treatment Educate patients, families, and caregivers on home programs and safety Collaborate with an interdisciplinary team (nursing, rehab, physicians, social work) Ensure accurate, timely, and compliant clinical documentation Maintain a safe, clean treatment environment and proper equipment use Support discharge planning and functional goal achievement for residents Participate in continuing education, in-services, and quality improvement initiatives Required Skills Therapeutic exercise implementation Gait and balance training Transfer and mobility training Geriatric rehabilitation Skilled nursing/long-term care rehab Functional assessment and documentation Use of assistive devices and durable medical equipment Patient and family education Electronic medical record (EMR) documentation Interdisciplinary team collaboration
