Job Description
Liberty Health seeks a compassionate Physical Therapy Assistant to support evidence-based, person-centered rehabilitation services. Working closely with licensed Physical Therapists, you will carry out treatment plans, guide therapeutic exercises, utilize mobility and assistive devices, and document patient progress. You'll educate patients and caregivers, promote safety and functional independence, and collaborate with an interdisciplinary behavioral health and medical team. Responsibilities Provide physical therapy interventions under direction of a licensed Physical Therapist Implement treatment plans including therapeutic exercise and functional training Assist patients with mobility, gait training, and use of assistive devices Monitor, record, and report patient progress and response to treatment Educate patients and caregivers on home exercise programs and safety Maintain accurate, timely clinical documentation in accordance with standards Collaborate with interdisciplinary healthcare and behavioral health teams Support evidence-based, person-centered and outcomes-focused care Ensure a safe treatment environment and proper equipment use Participate in quality improvement and program initiatives Required Skills Therapeutic exercise implementation Gait and balance training Manual therapy assistance Patient transfer and mobility techniques Use of assistive and adaptive equipment Clinical documentation and EHR use Knowledge of musculoskeletal and neurological rehab Infection control and safety practices Patient and caregiver education Interdisciplinary team collaboration
