Job Description
CenterWell Home Health seeks a compassionate Physical Therapist Assistant to provide in-home rehabilitative care. In this role, you'll implement treatment plans under the direction of a licensed Physical Therapist, assist with therapeutic exercises, gait and balance training, and patient education to promote safety, independence, and improved quality of life. You'll collaborate closely with an interdisciplinary care team, document patient progress, and communicate changes in condition. Responsibilities Provide in-home physical therapy interventions under the supervision and direction of a licensed Physical Therapist Implement established plan of care, including therapeutic exercises, gait training, balance and transfer training, and functional mobility activities Observe, measure, and document patient progress, response to treatment, and functional outcomes in a timely and accurate manner Communicate changes in patient condition or safety concerns promptly to the supervising Physical Therapist and clinical team Educate patients and caregivers on home exercise programs, safe mobility techniques, fall prevention, and proper use of assistive devices and equipment Reinforce patient and caregiver understanding of therapy goals, treatment techniques, and safety precautions within the home environment Collaborate with the interdisciplinary home health team to support coordinated, patient-centered care and optimal outcomes Travel to patient homes according to an assigned schedule while maintaining punctuality, professionalism, and infection-control standards Maintain compliance with state practice regulations, payer requirements, and Center Well Home Health policies and documentation standards Participate in case conferences, quality improvement initiatives, and ongoing training to support clinical excellence and professional growth Required Skills Home health physical therapy interventions Therapeutic exercise implementation Gait and balance training Transfer and mobility training Patient and caregiver education Home safety and fall-risk assessment Geriatric rehabilitation Pain management modalities within PTA scope Electronic medical record (EMR) documentation Clinical assessment of functional mobility within PTA role Care coordination and interdisciplinary communication Time management and route planning Infection control and standard precautions in the home Knowledge of state PTA practice regulations Use and adjustment of assistive devices (walkers, canes, wheelchairs)
