We partner with facilities throughout Colorado, including long-term care centers, skilled nursing facilities, and rehabilitation centers. Our providers play an important role in simplifying what can often be a complex transition-from hospital discharge to rehabilitation and, ultimately, back home. Position Summary We are seeking a compassionate and motivated Nurse Practitioner or Physician Assistant to join our growing team. The ideal candidate will have experience in hospital medicine, skilled nursing, long-term care, rehabilitation, or a related setting. This role requires strong clinical judgment, excellent communication skills, and the ability to work collaboratively with patients, families, physicians, and facility staff.
